2014 Senior Farmers’ Market Nutrition Program


The Hawaii Foodbank will be distributing a limited number of Senior Farmers’ Market Nutrition Program voucher booklets worth $50.00 at partner locations around the island. Our goal is to provide fresh and locally-grown fruits and vegetables for our seniors. Vouchers are redeemable with certified vendors at various farmers’ markets from
June 27October 26, 2014 (due to weather conditions).

Recipients must:

Age Verification (Bring 1):

Income Verification (Bring 1):

• Be 60 years of age or older; AND

• Have a household annual income of less than $24,827 for single person or $34,467 for a couple

• Current Hawaii Driver’s License
  and Social Security Card

• State ID and Social Security

• Certified Birth Certificate &
  photo ID

• Passport and proof of residency

• EBT Card

• Medicaid Card

• 2013Tax Return Form 1040

• 2014 Social Security Checks

• 2014 Pay Check Stubs

Bring completed application & required documents to a certification site below
(subject to voucher availability):
A senior must qualify every year and are limited to one set of vouchers per year.
